I believe PIE *kw can derive from :
- fundamental u in *ku reinterpreted as *kw in the ancestor of PIE,
leading to q in Chamito-Semitic.
- *k + *w (non fusing to q in CS)
- *k + *b (voiced not to be confused with PIE *b = p?)

An example is *gwor mountain which I consider is a derivative of *gab-al
"mountain"
Root *gab > PIE *gw-or
Cf. Archaic PIE form in French Cevennes < kab-anna "mountain".

Labio-velars are rare as suffixes because they entail _two_ suffixes : k +
w/b
