An edition by Madhav Deshpande was published in Japan:

Milindapanha-Atthakatha / by Thaton Mingum, Zetawun Sayadaw, alias U Narada Mahathera ; transcribed and edited by Madhav M. Deshpande. Tokyo : The International Institute for Buddhist Studies of The International College for Advanced Buddhist Studies, 1999 (Studia Philologica Buddhica Monograph Series XIII).

You may be able to get a copy of this from the publisher if still in stock: http://www.icabs.ac.jp/publication/2-7-2.html. Failing that a copy seems to be available from AddAll antiquarian books:
