I was able to make an IPA keyboard for myself (with mappings based roughly
on X-SAMPA values, given an underlying Dvorak layout) with little trouble.

Basically you're given a visualization of a blank keyboard and can assign
to each key whatever character you wish, and when finished you can export
an installer that will allow you to use the new layout as you would any
other Windows IME.
