I'm only now getting a chance to wade through back-messages on this
list, and please ignore this if your question has already been answered.

sa"nkhaaraa, vi~n~naa.na.m, bhavo are all nominative. The first is
nominative plural (formations), the others nominative singular. Note
that some of the terms are neuter-a nouns (thus ending in -a.m in
nominative), some are masc-a (-o ending), and feminine-aa (-aa ending,
as in ta.nhaa).

paccayaa is ablative, and the compounds in paccayaa are Tappurisa.
E.g. "ta.nhaapaccayaa upaadaana.m" is literally "from the condition of
craving, [there is (hoti is implied)] clinging". Simiilarly,
"upaadaanapaccayaa bhavo" - "from the condition of clinging, there is
becoming".
