Not sure if you're talking about specific to the United States or not, but:
In a piece of text under copyright, under U.S. law it continues to be
legal to quote or include portions of the text, for purposes of
writing a review, critique, response, research paper, etc. This is
called 'fair use'. The amount of text that may be reproduced varies
according to the size of the originating article, and your own
judgement; if a document is several hundred pages long, quoting an
entire page worth of material would most likely still be 'fair use';
for a three page document, quoting a third of the entire text would
clearly be pushing the boundries. The use that the reproduction is
being put to also affects wether or not it is fair use; most
educational and non-profit uses would easily be considered fair use,
whereas use in a commercial product would be more tricky.
For an image, any reprinting and distribution of an image under
copyright would be considered to be 'publishing' or reproducing the
image. I don't think that an image, even with attribution, can be
published in this way without permission from the copyright holder.
These standards (particularly what does or doesn't constitute 'fair
use') are somewhat subjective; if you have serious questions about
something that you are considering publishing, I would really
recommend you consult with a lawyer who focuses on copyright and
intellectual property issues. For a small quotation in an educational
work, there is much less scrutiny.
